SureFire Offers Assurant Dental Insurance
SureFire reserves the right to change plan terms and contribution amounts at any time. SureFire currently pays 75% of the HMO cost for Employees and 50% of the HMO costs for Dependents. SureFire will pay the same dollar amount that is currently paid for HMO toward the cost of PPO plans. Employees are responsible for any additional premium costs for PPO plans.
DMO
Assurant DMO provides most preventive care at no cost to members. The plan has no deductible. Preventive dental care provides x-rays and cleanings at no cost to the member. Note: Some procedures do require a co-payment at the time of service. There are no claim forms to fill out, and no annual maximums for most services. DMO also provides Orthodontic coverage and referrals to specialists. |

PPO
Assurant PPO deductible of $50/member per calendar year with maximum $2000. Members are free to use any licensed Dentist of their choice. However, using a network Dentist will generally result in a greater savings to members. |
