SureFire Offers Health Net and Kaiser Permanente Medical

SureFire reserves the right to change plan terms and contribution amounts at any time. SureFire currently pays 75% of the HMO cost for Employees and 50% of the HMO costs for Dependents. SureFire will pay the same dollar amount that is currently paid for HMO toward the cost of PPO plans. Employees are responsible for any additional premium costs for PPO plans.

Health Net HMO Benefits
Health Net HMO benefits are covered only when services are provided or coordinated by the primary care physician. Office visits only require $10 co-pay, and there is 100% coverage for most services. This plan offers a wide selection of hospitals and primary care physicians. |

Health Net PPO Benefits
Health Net PPO deductible of $500/member per calendar year with maximum of $3000 per member. A wide physician network is available. If using a PPO provider, Health Net will generally pay 80% after the deductible. If using a Non-PPO provider, Health Net will generally pay 60% after the deductible. |

Health Net Salud y Mas Benefits
Health Net Salud y Mas HMO benefits offer access to providers in the United States or Mexico’s SIMNSA Network. Office visits only require $15 co-pay within the U. S. Network, and only $5 co-pay within SIMNSA Network. There is a $250 co-pay for Hospital charges within the U.S. Network, and no charge within the SIMNSA Network. There is 100% coverage for most services within the SIMNSA Network, where as there is only 80% coverage within the U. S. Network. |

Kaiser Permanente Benefits
Kaiser Permanente benefits offer 100% coverage for most services with $10 co-pay for office visits. There is no deductible for inpatient hospital care, and $100 co-pay for emergency room visits. All services are provided through Kaiser Permanente facilities. Members are assigned a health care physician. |
